Core Responsibilities

The support you’ll provide

This role combines companionship, practical household support, errands, transportation, and careful attention to client safety.

Companionship & Social Interaction

  • Engage clients in conversations, reading, and hobbies.
  • Accompany clients on walks and social activities.
  • Plan and participate in recreational games or exercises.
  • Provide meaningful emotional and mental stimulation.

Daily Living & Household Assistance

  • Prepare nutritious meals and assist with hydration.
  • Perform light housekeeping like dusting, vacuuming, and sweeping.
  • Wash dishes, clean the kitchen, and manage laundry.
  • Change bed linens and keep living spaces organized.

Errands & Transportation

  • Drive clients to medical appointments and pharmacies.
  • Handle grocery shopping and run routine errands.
  • Escort clients to family gatherings or community events.

Health & Safety Monitoring

  • Provide timely verbal medication reminders.
  • Observe and track changes in physical or mental behavior.
  • Log daily activities and report issues to supervisors.
  • Maintain a clean, hazard-free environment to avoid falls.

Qualifications

Who we’re looking for

Graceful Connections Home Care is looking for dependable, compassionate individuals who are ready to support clients with patience, respect, and professionalism.

“Prior experience is preferred, but we are willing to train the right person who brings compassion, reliability, and a genuine desire to help others.”

Education

High school diploma or equivalent certification.

Experience

Prior experience preferred, but training may be provided.

License & Transportation

Valid driver’s license with a clean driving record and access to a reliable, insured personal vehicle.

Background Check

Must successfully clear criminal and background checks.

Health Screenings

Proof of negative TB test may be required.
