Yanique Lewis, MS, RN, AGNP-C, brings nearly two decades of diverse, hands-on healthcare experience to her role as Co-founder of Graceful Connections Home Care. Her career is defined by deep clinical expertise and a profound commitment to patient-centered care. With over 20 years of clinical experience in geriatrics and community health, Yanique has dedicated her career to advocating for elderly and disabled populations.
Yanique began her healthcare journey on the front lines, spending 10 years as a Home Health Aide (HHA) and Certified Nursing Assistant (CNA). During this time, she specialized in caring for the elderly and individuals living with Alzheimer’s and dementia. This foundational experience instilled in her deep empathy and a thorough understanding of the unique challenges families face when navigating home-based care.
Driven to expand her impact, she earned her nursing degree and served for 8 years as a Registered Nurse (RN) in a high-acuity Surgical Trauma unit. In this fast-paced environment, she mastered critical thinking, crisis management, and complex clinical interventions.
She realized that many patients thrive best when they can recover or age in place, surrounded by the comfort of their own homes. Driven by this realization, she launched Graceful Connections Home Care to deliver high-quality, compassionate care directly to the community. As co-owner, she leverages her full spectrum of experience to ensure Graceful Connections Home Care delivers a high standard of safe, skilled, and dignified care directly to patients in the comfort of their homes.
Today, as a board-certified Adult-Gerontology Nurse Practitioner, Yanique bridges the gap between advanced clinical leadership and compassionate, grassroots care. Under her leadership, the agency focuses on highly individualized care plans, rigorous caregiver training, and maintaining strict compliance with New York State Department of Health regulations. She is deeply committed to serving the local Suffolk and Nassau community by matching patients with caregivers who meet their specific home, cultural, and personal needs.

Tiffany Kelly, BSN, RN, is the Co-Founder of Graceful Connections Home Care and a compassionate registered nurse devoted to helping clients and families feel supported, safe, and truly cared for. She co-founded Graceful Connections Home Care with a heartfelt mission: to ensure individuals can remain in the comfort of their own homes while receiving attentive, personalized care that enhances their well-being and preserves their independence.
With a Bachelor of Science in Nursing and hands-on experience in hospital and rehabilitation settings, Tiffany brings both clinical expertise and genuine empathy to every client interaction. Her background in medical-surgical nursing, telemetry, patient assessment, chronic disease management, medication management, and care coordination allows her to anticipate needs and provide thoughtful, proactive care. Clients and families can feel confident knowing they are supported by someone who listens, understands, and prioritizes their unique circumstances.
Tiffany’s commitment to excellence extends beyond her current practice. She is pursuing a Master of Science in Nursing as an Adult-Gerontology Nurse Practitioner (AGNP), further strengthening her ability to deliver advanced, high-quality care. This dedication ensures that clients benefit from the latest knowledge and best practices, helping them achieve better health outcomes and a higher quality of life.
